TRANJUGULAR LIVER BIOPSY

PREOP:

  • Preference to room 2-3
  • Outpatient procedure

ESTIMATED TIME: 20-30min

 

Equipment Always Necessary:

  • Micropuncture
  • TJ liver biopsy set
  • 10Fr TIPS sheath
  • Fogarty cahteter
  • Formaline cup

If Paracentesis done at the same time:

  • US with the P4 probe prepped (in this case both probes will be prepped)
  • Safe-T-centesis cahteter
  • 4-6 Vacuum bottles

PREP:

  • POSITION: Supine
  • Right IJ is the preferred site
  • Need US and probe cover for both probes
  • DRAPE: Thyroid drape - neck
  • Chest-Breast drape - abdomen
  • Position screens in a place that can be seen
  • Set up pressure measurements

 

Dressing:

  • No dressing. Dermabond only.

PEARLS:

  • Frequently paracentesis is performed at the same time. 
  • This procedure not only includes the biopsy, but also the measurement of the portal pressures.
  • Portosystemic gradient - PSG=PORTAL PRESSURE - CENTRAL PRESSURE. If >12 indicates portal hypertension.
