PREOP:
- Preference to room 2-3
- Outpatient procedure
ESTIMATED TIME: 20-30min
Equipment Always Necessary:
- Micropuncture
- TJ liver biopsy set
- 10Fr TIPS sheath
- Fogarty cahteter
- Formaline cup
If Paracentesis done at the same time:
- US with the P4 probe prepped (in this case both probes will be prepped)
- Safe-T-centesis cahteter
- 4-6 Vacuum bottles
PREP:
- POSITION: Supine
- Right IJ is the preferred site
- Need US and probe cover for both probes
- DRAPE: Thyroid drape - neck
- Chest-Breast drape - abdomen
- Position screens in a place that can be seen
- Set up pressure measurements
Dressing:
- No dressing. Dermabond only.
PEARLS:
- Frequently paracentesis is performed at the same time.
- This procedure not only includes the biopsy, but also the measurement of the portal pressures.
- Portosystemic gradient - PSG=PORTAL PRESSURE - CENTRAL PRESSURE. If >12 indicates portal hypertension.
